The website form is a request for a private cash physical therapy visit with Ben Graves, PT, DPT. It is not online check-in for Lillian M. Hudspeth Memorial Hospital and it is not insurance registration.

Do not use this form to send medical history, pain details, imaging results, medication lists, or emergency concerns. If this is an emergency, call 911 or go to the nearest emergency room.

To schedule hospital outpatient physical therapy, call (325) 387-1291. Do not submit hospital patients through this website.

Submitting the form does not reserve a final appointment. The visit is confirmed only when Ben replies with a time and place.

Payment is due at the visit unless we agree otherwise in writing. - Initial evaluation: $100 (60 minutes) - Treatment: $75 per hour or $40 for 30 minutes - San Angelo visits: $20 travel fee You are responsible for your own green fees, cart, driving-range bucket, gym membership, or day pass. After a cash physical therapy visit, you may request an itemized superbill to submit to your health plan, HSA, or FSA for possible out-of-network reimbursement. This practice does not bill insurance, does not contact payers on your behalf, and cannot guarantee coverage or payment.

Give at least 24 hours’ notice to change or cancel. Late cancel or no-show may forfeit the reserved time. Repeat late cancels may lose access to online requests.

Physical therapy does not guarantee a specific result. Evaluation findings and a plan of care are created after we see you in person.

Clinical records are created after the visit. Website scheduling data is kept only as long as needed for scheduling, follow-up, legal, and professional requirements, then deleted or stored securely.
