This form is for private cash-based physical therapy or performance training with Dr. Ben Graves, PT, DPT, CSCS, BRM. Choose physical therapy or sports performance training. Performance training is not a PT visit. This is a request — not a confirmed appointment — until Ben replies. A no-show, or a cancel within 1 hour of the session, may be charged 50% of the visit or session cost, due before the next session.

You are responsible for your own green fees, cart, or driving-range bucket. Pay the course or range directly — those fees are not included in the visit price.
Graves Performance Therapy, PLLC does not enroll with or bill Medicare for outpatient physical therapy. Medicare rules for physical therapists differ from physician “opt-out” rules. If you are a Medicare beneficiary, do not book online. Contact us first at [email protected] so we can determine whether any requested service is appropriate to schedule and how federal rules apply. Sports performance training is athletic conditioning and is not billed as physical therapy. After a cash physical therapy visit, you may request an itemized superbill to submit to your health plan, HSA, or FSA for possible out-of-network reimbursement. Graves Performance Therapy, PLLC does not bill insurance, does not contact payers on your behalf, and cannot guarantee that any amount will be covered. Superbills apply to physical therapy visits only — not sports performance training. Private physical therapy may be by Texas Direct Access (up to 30 consecutive calendar days without a physician referral) or by physician referral. Either way, private visits are cash-based. Graves Performance Therapy, PLLC does not bill insurance for services provided. Bundled visit packages are available. Specifics are discussed with Ben directly.
