This is a request — not a confirmed appointment — until Ben replies. LMH Outpatient PT is scheduled by calling the hospital, not on this form. Please give at least 24 hours’ notice to change or cancel a visit.

You are responsible for your own green fees, cart, or driving-range bucket. Pay the course or range directly — those fees are not included in the visit price. For a visit at LMH Wellness Center a membership is needed or there is a $5 charge for a day pass.

You are responsible for your own green fees, cart, or driving-range bucket. Pay the course or range directly — those fees are not included in the visit price. Green fees are about $20 weekday / $23 weekend for 18 holes. Confirm at the shop — (325) 387-3680.
Graves Performance is a cash-based, out-of-network provider. We do not accept or bill private insurance or Medicare. If you are a Medicare beneficiary, please contact us directly prior to booking to ensure compliance with federal wellness guidelines. After a cash physical therapy visit, you may request an itemized superbill to submit to your health plan, HSA, or FSA for possible out-of-network reimbursement. Graves Performance Therapy does not bill insurance, does not contact payers on your behalf, and cannot guarantee that any amount will be covered. Superbills apply to physical therapy visits only — not sports performance training.
