What does a private Evaluation include?

The first visit (PT evaluation or performance training) is 60 minutes and $150. Follow-up PT or performance sessions are $150 for 60 minutes. Bundled visit packages are available; specifics are discussed with Ben directly.

Do I need a doctor’s referral?

Accepted Answer

Private physical therapy may be by Texas Direct Access (up to 30 consecutive calendar days without a physician referral) or by physician referral. Either way, visits are cash-based. Patients receive the Board-required Direct Access disclosure before treatment without a referral. Performance training is not physical therapy, is not medical treatment, and does not use Direct Access paperwork.

I have Medicare. Can I book online?

No. Graves Performance Therapy, PLLC does not enroll with or bill Medicare for outpatient physical therapy. Medicare rules for physical therapists differ from physician “opt-out” rules. If you are a Medicare beneficiary, do not book online. Email ben.graves@gravespt.com first so we can determine whether any requested service is appropriate and how federal rules apply. Sports performance training is athletic conditioning and is not billed as physical therapy.

What is the cancellation policy?

A no-show, or a cancel within 1 hour of the session, may be charged 50% of the visit or session cost, due before the next session. You are responsible for fees at the time of service unless we agree otherwise in writing.

Question 5

Can I use insurance or an HSA for a private visit?

Accepted Answer

This practice does not bill insurance. After a cash physical therapy visit you may request a superbill and choose to submit it to your health plan, HSA, or FSA for possible out-of-network reimbursement. Superbills are for physical therapy visits only — not sports performance training. Coverage is not guaranteed.

What extra fees should I expect?

San Angelo visits add a $20 travel fee. You are responsible for your own green fees, cart, or driving-range bucket at Sonora Golf Club. Bundled visit packages are discussed with Ben directly.

Is Graves Performance Therapy mobile?

Accepted Answer

Yes. Ben sees clients at Sonora Golf Club (1612 Golf Course Rd, Sonora, TX) and also comes to you on the golf course, driving range, court, or field in Sonora and San Angelo, Texas.

Is this cash-based physical therapy?

Yes. Private visits are cash-based and this practice does not bill insurance. An evaluation or initial session is $150 for 60 minutes, and treatment or training sessions are $150 for 60 minutes. San Angelo visits add a $20 travel fee. You can request a superbill after a PT visit.

Can I see a physical therapist in Texas without a referral?

Accepted Answer

Yes. Under Texas Direct Access (H.B. 4099, Tex. Occ. Code §453.301), you can be treated without a physician referral for up to 30 consecutive calendar days. A referral is required to continue treatment beyond that.

What should I expect at my first visit?

Accepted Answer

Your first visit is 60 minutes. Ben assesses how you move, finds where the problems and deficits come from, and builds a plan with treatment and exercises you can do between visits.

Question 11

Do you work with golfers in San Angelo and Sonora?

Accepted Answer

Yes. Golf rehab and performance visits happen on the course or range, and address low back, trail shoulder, lead-hip and elbow pain in the swing. You pay your own green or range fees.
